Pastoral/Parent Recommendation – Print to be completed by pastor/parent

• All pastoral and parental recommendations must be submitted by the pastor/parent, not the prospective intern.

• The application deadline for prospective interns who live outside Kootenai County is July 8th, 2012.

• The application deadline for Kootenai County residents is August 5th, 2012.

• There is a $30 application fee (pay online or make checks payable to New Life Community Church, write “intern application fee” in the memo line and mail to the address below)

• There is a $50 late application fee (in addition to the normal $30 application fee)
