Pastoral/Parent Recommendation – Print to be completed by pastor/parent

• All pastoral and parental recommendations must be submitted by the pastor/parent, not the prospective intern.

• The application deadline is August 5th 2012.

• There is a $30 application fee   

• Applications submitted after August 5th 2012 will be subject to a $50 late application fee (in addition to the normal $30 application fee)
